Program Summary

One Day AT A Time (ODAAT) is a Program Partner of the Urban Affairs Coalition (UAC). ODAAT houses men and women who are afflicted by addiction and homelessness. Case management services and prevention for HIV/AIDS is also offered at select locations. Through our holistic array of services, ODAAT consumers can expect the support of staff to bring systems together to achieve positive outcomes. Staff offers support in the following areas: recovery coaching and mentoring, linkage to health and social services, transitional housing, and community outreach.

Position Summary

One-Day-At-A-Time (ODAAT), a program partner of the Urban Affairs Coalition (UAC), is currently looking for an individual to fill the full-time position of Youth & Family Assessor.

The Youth & Family Assessor can expect to deliver supports and programs consistent with recovery planning and resiliency building. Youth/young adult life plans established with youth/young adult will target to enhance their capacity to function in all aspects of community living. Responsibilities include developing and implementing life skills programs based on youth/young adult life plans, providing case management to individuals on caseload, providing appropriate personal and physical care, crisis response, counseling and emotional support, where appropriate services may be extended to children in school or home with their families.

The ODAAT Youth & Family Services Department works with at-risk youth and young adults to enable them to grow as individuals and become productive members of their community in- and out-of-school. This position is an exciting opportunity to work as part of a comprehensive youth engagement initiative which includes early intervention, prevention, and program development activities that are monitored through assessments and evaluations.

Key Duties & Responsibilities:

  • Provide clinical support and case management under the Harm/Risk reduction and Trauma-informed service spectrum delivering ODAAT services to at-risk youth and young adults and families
  • Provide brokerage/resourcing and advocate for youth and families using evidence-based practices. Provide general information and referral services to the youth for mental health, recreation, housing, employment, education, health and other services
  • Work collaboratively with youth/young adults, parents, youth service providers, schools, city staff and law enforcement to develop effective service delivery plans that ensure that youth have access to programs, services and opportunities needed for healthy development by:
    1. Engaging youth/young adults to identify issues, needs, goals aspirations, skills, abilities and services needed as well as barriers to accessing these services
    2. Providing youth with information, contacts and referrals needed to facilitate access to programs, services and opportunities
    3. Engaging parents through existing parent groups, faith communities, etc., and provide education and information on issues related to parenting, etc.
    4. Organizing and participating in public education seminars, workshops, groups and training for youth/young adults, parents and service providers
  • Initiate, develop and implement community outreach activities with youth/young adults and communities in Philadelphia. Facilitate connections between youth and their neighborhood and promote awareness of the positive role of youth in the community
  • Develop an understanding of neighborhood demographics, and location of services, programs and community assets available in the local neighborhood
  • Required to submit data to DDAP via the PA WITS System within 2 weeks of the date of service delivery
  • Provide motivational counseling, brief interviewing techniques and have knowledge about the ideology behind community violence and addiction
  • Completion of the American Society of Addiction Medicine (ASAM) assessment and all service interventions (participant, group, family) required
  • Identify emerging community issues, service gaps and opportunities for action to build on community capacity
  • Other agency related duties as required or assigned

Education, Knowledge, Skills & Abilities:

  • Master’s degree required; Social Work, Education or Related degree.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, etc.).
  • Proven clinical skills required including case note development, client assessments and
    file maintenance.
  • Proven team leader capable of supervising facilitators and classroom activities.
  • Excellent verbal and written communication skills.
  • Knowledge of and sensitivity to the life experiences and needs of at risk youth/young adults.
  • Demonstrated ability to work cooperatively and negotiate effectively within a multi-
    disciplinary team of service providers.
  • Demonstrated relevant experience.
  • Clear understanding of and commitment to a harm reduction approach.
  • Clear understanding of and commitment to supporting diverse communities and
    cultures.
  • Bilingual preferred, but not required.
  • Must be able to prioritize and meet deadlines.
  • Able to develop rapport, provide information, and refer with sensitivity to cultural
    issues.
  • Ability to work and communicate with staff, clients and their families, donors, vendors, community agencies, etc. to meet their needs in a polite, courteous and cooperative manner, yet appropriately assertive as situation may demand.
  • Display a high level of initiative, effort and commitment towards completing assignments efficiently.
  • Possess excellent time management skills and the ability to work with minimum supervision.
  • Ability to transport individuals as needed, and travel to various sites and locations occasionally.
  • Knowledge of budget management preferred.
  • Able to meet a flexible work schedule including summer hours, and occasional evenings
    and weekends, and availability by telephone and/or cell phone during non-work hours.
  • Other agency related duties as required or assigned.

 

LICENSES & CERTIFICATIONS

  • Valid Pennsylvania driver’s license preferred.

 

PHYSICAL REQUIREMENTS

  • Able to lift/move weight up to 25lbs.
  • Vision adequate to perform the responsibilities and functions of the job efficiently.
  • Must be able to sit for three (3) or more hours per day.
  • Must be able to stand for two (2) or more hours per day.
  • Must be able to operate a computer/hand-held digital device for more than two (2) hours per day.

 

WORK ENVIRONMENT

The employee will work in an office and public, service-delivery, and school environment. Hectic situations can occur in the public area of the facilities characteristic to working with the youth and the related concerns and situations related to at-risk youth/young adults. Potential exists for volatile and unpredictable situations to occur. The requirements listed in this document are the minimum levels of knowledge, skills, or abilities.
