Program Summary: 
SELF/UAC operates a variety of federal, state and locally funded programs and services that serve single adult populations who have barriers to independent living including, but not limited to, homelessness, drug and alcohol addictions and mental health.  Programs/services range from 24 hour residential emergency, transitional and recovery housing, to daytime and/or evening short-term shelter and street outreach.  Program sizes vary from 20 to 230 participants and are located in the North and West Philadelphia communities.

Key Duties & Responsibilities: 
JOB RESPONSIBILITIES:
  • Maintain a caseload of at least thirty-five (35) program participants.
  • Provide technical assistance and training on case management functions and policies, as requested, for new Case Managers.
  • Assist Supervisor and Director in providing comprehensive reviews of Case Managers data entry in the Office of Homeless Services Housing Management Information System (HMIS).
  • Conduct Quarterly Monitoring Reviews of the required funding database usage to ensure that Case Reviews are completed in accordance with the funders’ guidelines and submit findings to Supervisor for follow-up.
  • Provide quality reviews of services via the funding source required database rendered by Client Service Case Managers and maintain the HMIS inbox dialogue, as requested as the Master Assessor of the system.
  • Assist Client Service Case Managers in areas of provision of services to clients such as needed referrals (i.e. mental health, drug & alcohol, physical and housing).  
  • Provide assistance and consult with Case managers regarding difficult cases. 
  • As requested, assist with the preparation of compliance protocols submitted by Client Service Case Managers to ensure that they are in compliance with OHS standards.
  • Provide the Director of Client Service Case Management with quarterly reports on assistance rendered to Case Managers, and provide suggestions for revising practices and procedures, if deemed necessary.
  • Conduct periodical reviews of housing applications submitted via the OHS system.
  • Evaluate and provide training when needed to ensure effectiveness of services given by Case Managers.
  • Perform related work as requested.
Education, Knowledge, Skills & Abilities: 
QUALIFICATIONS:
  • Bachelor’s degree in a human service field, including, but not limited to Social Work from an accredited University or College.
  • A combination of a minimum of three years acceptable training and experience in homeless, mental health, and/or addiction treatment services.
  • Must be well organized, detail-oriented, tasks-driven and able to work independently and set daily priorities by maintaining a weekly as well as a monthly schedule.
  • Must possess strong verbal and written communication skills and willingness to assist whenever needed.
  • Must have a valid driver’s license and motor vehicle record.
  • Criminal Background clearance will be required.
EXPERIENCE: Three years of experience working in programs with residential facilities/properties and providing services to individuals who may have challenges that include homelessness, substance abuse and mental health, or have other barriers to self-sufficiency.
