Case Management and Social Services
Position Summary: 
UAC/CFH is looking for a qualified Intake Support Coordinator. Under the supervision of the Director of Case Management and Social Services, the Intake Support Coordinator will assist and support the Director of Case Management and Social Services to achieve full compliance with the City of Philadelphia's Office of Homeless Services program. The Intake Support Coordinator will carry a full case load and ensure the provision of intake and planning related services as prescribed by the city of Philadelphia’s Emergency Housing Standards. Also, this person will be responsible for arranging Intake, Orientation, and Continuing of Care and Follow up.
Key Duties & Responsibilities: 
Conduct intake and assessment for consumers, schedule appointments, make recommendations, coordinate services, compile reports, and keep necessary records. • Establish room assignment, orientate consume r(s) to facility, and coordinate appropriate room clustering given initial assessment. • Create, maintain, update, and produce client charts, case notes, service plans, confidentiality agreements, and all other necessary documentation as required by the funder and administration; • Ability to assist the consumers in obtaining birth certificate, ID(s), Medical Insurance and proof of change of address. • Establish and maintain referral relationships with services providers. • Arrange DPA, Social Security, Food Stamps, and Medical assistance appointments to ensure timely and continued receipt of benefits. • Facilitate and schedule psychiatric evaluations, medical management appointments, and out-patient treatment services. • Identify social service resources for consumers to make appropriate referrals. • Make appropriate referrals to educational and vocational training programs. • Attend trainings as mandated by compliance requirement s, case manager director, and funding agency. • Ability to organize time, prioritize assignments, including ability to work flexible hours outside of routine 9am to 5pm office schedule as required. • As time permits and according to an agreed schedule with case manager director attend and participate in company outreach events which support programs. • Attend all mandatory staff functions, based on pre-authorized schedule with case manager director. • Must be knowledgeable in Excel, Power Point and Word. • Other duties as assigned
Education, Knowledge, Skills & Abilities: 
Bachelor’s degree in human services or related fields and 2-3 years of case management experience is required. An employment history of working with at-risk populations including but limited to; the chronically homeless, those suffering with addiction and/or mental health issues, those with a criminal history, intellectual disabilities, VETs, medically fragile and adults 18 years or older is preferred. • Must be computer literate (Microsoft Office products). A working knowledge on how to create and produce case notes, service plans, and Outlook is preferred. REQUIRED KNOWLEDGE, SKILLS AND ABILITIES: • Knowledge of basic principles, practices and procedures involved in shelter facility • Knowledge of the physical and emotional characteristics, manifestations of alcohol and drug dependency and mental health • Ability to work effectively with people and aid them in utilizing available resources • Ability to establish and maintain effective working relationships with residents , other members of staff, outside agencies, shelter system and the general public • Ability to communicate effectively , both verbally and in writing • Ability to initiate and maintain contacts with social service agencies, educational and vocational programs, etc. • Ability to organize a large case load, client charts, and an extensive information system • Ability to accept supervision to effectively learn case management functions that correspond with intake position
