We offer a broad range of essential housing, social service, drug and alcohol treatment, vocational and employment programs, all designed to help individuals achieve self-sufficient and productive lifestyles. We provide these services to more than 7,000 individuals within the Philadelphia area.

Key Duties & Responsibilities: 

JOB RESPONSIBILITIES:

• Maintain caseload of approximately 40-50 consumers.

• Identify and screen prospective consumers in need of alcohol or other drug services.

• Perform initial matrix assessment of each consumer upon admission to shelter (including identifying data physical, mental, substance abuse, employability and income history, etc).

• Assist consumer with obtaining identification (i.e. state id, birth certificates, and social security cards) to establish basics to obtain needed services.

• Maintain the ability to schedule consumers according to OSH standards task importance (i.e. overdue tasks completion, newly assigned consumers and pending task consumers).

• Maintain the ability to be in compliance with HMIS task completion timeliness (i.e. initial contact, case management meetings, developing housing plans, completing CARES Consents, updating of service plans every 60-day, etc).

• Meet with consumer within 3-5 days of case assignment and conduct initial data gathering to develop client record,

• Complete client master within 20-days of case assignment to develop individualized service plan that is required within 30-days of case assignment date.

• Meet with consumer at least 1x monthly & document progress using goal step status.

• Develop and maintain consumer case records, compile history, maintain accurate and complete documentation of service activity, interaction and progress as it occurs.

• Arrange/schedule medical/ psychological, substance evaluation, etc for (new/needy) consumers.

• Complete and submit housing referrals after 30-days of consumer assessment and/or initial service plan completion and/or contingent upon consumer meeting housing program eligibility requirements.

• Maintain current appointment and roster case load submitting to supervisor weekly

• Maintain and ensure accuracy of client records according to OSH standards and protect the confidentiality of records and file security.

• Assist the Client Services Supervisor to conduct monthly file audits.

• Conducts and initiate the process and procedures involved in the preparation and completion of all documents and forms to the point of acceptance or rejection of prospective consumers in collaboration with OSH guidelines.

• Participate in Exchange Meetings and Client Progress Reviews (CPR) process.

• Coordinate positive consumer movement from the shelter with emphasis placed on addressing core issues of homelessness, enhancing consumer’s income status and expeditious facilitation of consumer movement from emergency shelter into appropriate housing resources (transitional and permanent housing) that are available.

• Assist in the collection of urine samples.

• Help consumers to identify, define and resolve personal problems and outline on service planning.

• Coordinate all efforts to provide consumer related services with appropriate support services and residential staff, other social service providers and member of involved outreach teams.

• Coordinate consumer’s transportation via public/outreach transport as necessary for appointments.

• Serve as an advocate for those consumers receiving/ not receiving services from outside resources.

• Maintain an awareness of the health, welfare and treatment and legal status of each consumer in the program.

• Report all pertinent information to Client Services Supervisor.

• Meet all internal and external reporting requirements, i.e. Weekly Reports, PBM’s, OSH and POS’s etc. in a timely manner.

• Work closely with personnel of other community agencies who are actively involved with programs consumers.

• Conduct individual and group counseling using a variety of techniques to assist consumer in gaining independent living plan goals.

• Respond with flexibility in cooperation with emergency request, changes in work schedule or assignment.

 

Education, Knowledge, Skills & Abilities: 

MINIMUM QUALIFICATIONS:

• A Bachelor of Arts or Science degree from accredited university or college.

• Acceptable training experience in homeless shelter, mental health and addictive diseases.

• Participation in general staff meetings, department meetings and training as directed.

• Must be well organized, detail-oriented, tasks-driven and able to work independently and set daily priorities by maintaining a weekly as well as a monthly schedule.

• Must possess strong verbal and written communication skills and willingness to assist whenever needed.

• Must have valid driver’s licensing and motor vehicle record.

• Criminal and Child Abuse clearance will be required.
